Ayurvedic Ophthalmology - Classical Foundations and Modern Evidence

Speaker:Welcome to the show. Today, we're looking at Ayurvedic ophthalmology, its classical
Speaker:foundations, and what modern evidence actually supports. My guest is
Speaker:Dr. Saikumar Gandapodi,
Speaker:a board-certified doctor of Oriental medicine and Ayurvedic practitioner and
Speaker:the founder of Netra Eye Institute. Doctor, thanks for being here.
Speaker:Thanks for having me. This is a topic I care about, partly because it's easy to
Speaker:romanticize and easy to dismiss.
Speaker:-I'd rather do neither. -Let's start there.
Speaker:Ayurveda has a long tradition around the eye.
Speaker:How old are we talking, and what did those early physicians actually describe?
Speaker:The classical reference point is the Sushruta Samhita,
Speaker:a surgical text usually dated to the early centuries of the Common Era. It devotes
Speaker:real attention to the eye, describing dozens of eye disorders and even a
Speaker:procedure for cataract called couching, where the clouded lens is displaced out of
Speaker:-the visual axis. -Couching,
Speaker:that's a genuine historical surgical technique, not folklore.
Speaker:Correct. It's a real procedure, and it was practiced for centuries across many
Speaker:cultures.
Speaker:By modern standards, it's obsolete and carries high complication rates, so I
Speaker:mention it as history, not as something anyone should do today.
Speaker:Modern cataract surgery is vastly safer and restores vision properly.
Speaker:That's a good framing. So how did they explain disease itself?
Speaker:-What was the conceptual model? -The core framework is the three doshas,
Speaker:vata, pitta, and kapha.
Speaker:I want to be precise here.
Speaker:These are not chemicals or cell types. They're an interpretive language for
Speaker:patterns, movement, metabolism, and structure. I treat them as a conceptual
Speaker:-parallel, not a scientific equivalent. -Can you give an example of how that maps
Speaker:even loosely onto something we'd recognize today?
Speaker:Take pitta, which is associated with heat and transformation.
Speaker:Classically, inflammatory, red, burning eye conditions were often read as pitta
Speaker:patterns.
Speaker:In modern terms, we talk about inflammation and cytokine activity.
Speaker:The overlap is suggestive, but I won't pretend pitta equals a specific
Speaker:inflammatory pathway.
Speaker:It's a rough map, not a lab result.
Speaker:You also hear the term rakta dhatu in Ayurvedic eye discussions.
Speaker:What does that refer to?
Speaker:Rakta dhatu roughly translates to the blood tissue,
Speaker:and Ayurveda considered healthy blood essential to healthy eyes.
Speaker:Loosely, that echoes our modern understanding that ocular blood flow and
Speaker:vascular health matter for the retina and optic nerve.
Speaker:Again, a parallel, not a one-to-one translation.
Speaker:Let's talk about your own work.
Speaker:You founded Netra Eye Institute, and you practice something you call Netra
Speaker:Restoration Therapy.
Speaker:What is it?
Speaker:Netra Restoration Therapy, or NRT,
Speaker:is an integrative approach that tries to support eye health through several
Speaker:biological pathways at once
Speaker:rather than a single target.
Speaker:Depending on the condition, that can mean supporting ocular blood flow,
Speaker:reducing oxidative stress,
Speaker:calming inflammation,
Speaker:and attending to whole body factors like nutrition and sleep.
Speaker:I want to be clear with listeners about what that is and isn't.
Speaker:Please hold me to it.
Speaker:NRT is an adjunct.
Speaker:It complements standard ophthalmic care. It does not replace it.
Speaker:I don't make cure claims. I don't guarantee outcomes. And if someone needs
Speaker:surgery, drops, or injections, they should get them.
Speaker:What I offer sits alongside that.
Speaker:So why the multi-target philosophy at all? Why not just find the one thing that
Speaker:-works? -Because many chronic eye diseases aren't
Speaker:single cause.
Speaker:Something like dry eye disease involves tear film instability, inflammation, and
Speaker:ocular surface stress together.
Speaker:Ayurvedic formulas are interesting here precisely because they're multi-component.
Speaker:One preparation may contain many phytochemicals hitting several pathways,
Speaker:which is what network pharmacology tries to describe.
Speaker:Let's get concrete on the botanicals. Which Ayurvedic herbs actually have some
Speaker:modern data behind them for the eye?
Speaker:The most studied is probably Triphala, a three-fruit combination.
Speaker:There's laboratory work showing it's rich in antioxidant polyphenols, and small
Speaker:clinical studies have looked at Triphala eye wash, sometimes called Triphala
Speaker:Ghrita, for ocular surface and dry eye symptoms.
Speaker:The signals are encouraging, but the trials are small and often not blinded.
Speaker:-So promising, but not settled. -Exactly. I'd call it emerging, not
Speaker:established.
Speaker:I wouldn't tell a patient triphala treats their eye disease. I'd say there's early
Speaker:evidence it may support ocular surface comfort, and we need larger, better
Speaker:controlled trials before making stronger statements.
Speaker:What about the antioxidant angle more broadly?
Speaker:Oxidative stress comes up a lot in eye disease.
Speaker:It's genuinely relevant for certain conditions.
Speaker:The one place with strong human evidence is age-related macular degeneration, where
Speaker:the AREDS and AREDS2 trials showed a specific antioxidant and zinc formulation
Speaker:slowed progression in intermediate to advanced cases.
Speaker:That's conventional nutritional science, not Ayurveda, but it validates the general
Speaker:principle that oxidative stress matters in the retina.
Speaker:Does that give Ayurvedic antioxidant herbs a plausible mechanism then?
Speaker:Mechanistically, it's plausible that antioxidant-rich botanicals could
Speaker:contribute,
Speaker:but plausible is not proven.
Speaker:We don't have AREDS-level trials for Ayurvedic formulas in macular
Speaker:degeneration,
Speaker:so I'll say the mechanism is reasonable and the specific clinical proof isn't
Speaker:there yet. I try not to blur those two.
Speaker:Let's talk about turmeric, since everyone asks.
Speaker:Curcumin,
Speaker:where does it stand for eyes?
Speaker:Curcumin is a real anti-inflammatory and antioxidant compound in the lab,
Speaker:and it's been studied in eye contexts, mostly in cell and animal models.
Speaker:The big practical problem is bioavailability.
Speaker:Ordinary curcumin is poorly absorbed, so translating lab effects into the eye in
Speaker:humans is genuinely uncertain.
Speaker:I treat it as hypothesis generating.
Speaker:That bioavailability point seems important and often skipped.
Speaker:It's the difference between a nice Petri dish result and something that actually
Speaker:reaches the target tissue in a person.
Speaker:A compound can look powerful in vitro and do very little in vivo.
Speaker:Any honest integrative practitioner has to keep asking,
Speaker:"Does it get where it needs to go,
Speaker:and at what dose?"
Speaker:Ayurveda also emphasizes the whole body, not just the organ. Is there a scientific
Speaker:basis for connecting systemic health to the eye?
Speaker:For some conditions, absolutely. Dry eye and ocular surface inflammation are
Speaker:influenced by systemic inflammation, hormones, and nutrition.
Speaker:There's also growing interest in the gut-eye axis,
Speaker:the idea that the gut microbiome influences ocular surface immunity.
Speaker:I'd stress that it's early signs,
Speaker:more association than proven causation right now.
Speaker:Ayurveda would have framed that as digestion affecting the eyes long before
Speaker:-we had a microbiome. -It would, through concepts like agni,
Speaker:digestive fire, and ojas, a kind of vitality.
Speaker:Those are elegant metaphors, and sometimes they point in a direction modern research
Speaker:later explores.
Speaker:But a metaphor pointing in a good direction is not the same as it being
Speaker:validated.
Speaker:I hold both ideas at once.
Speaker:There's a classical Ayurvedic eye practice called netra tarpana,
Speaker:where medicated ghee is pooled over the eye.
Speaker:What's your honest read on it?
Speaker:Netra tarpana is popular for dry, tired eyes,
Speaker:and patients often report it feels soothing.
Speaker:The formal evidence is thin,
Speaker:mostly small studies and traditional use.
Speaker:There's also a real safety consideration,
Speaker:sterility, and what's actually in the preparation.
Speaker:So I'd say it may offer symptomatic comfort for some, but it needs proper
Speaker:study and careful hygiene,
Speaker:and it's not a treatment for eye disease.
Speaker:Safety is a good thread.
Speaker:What are the genuine risks people should know about with herbal or traditional eye
Speaker:treatments?
Speaker:The main ones are contamination, incorrect preparation, and putting non-sterile
Speaker:substances near the eye,
Speaker:which can cause infection or surface damage. There's also the risk of someone
Speaker:using an herb instead of a proven therapy and losing time in a condition where
Speaker:timing matters, like glaucoma or a retinal problem.
Speaker:That delay is the danger I worry about most.
Speaker:How do you personally decide what makes it into your practice versus what you set
Speaker:aside?
Speaker:Three filters.
Speaker:Is there at least plausible mechanism?
Speaker:Is there some human safety data?
Speaker:And does it stake clearly in the adjunct lane?
Speaker:If a practice fails any of those, I leave it out, or I'm upfront that it's
Speaker:traditional and unproven.
Speaker:And nothing I do interferes with a patient's ophthalmologist.
Speaker:Where do you think conventional ophthalmology, which is excellent at what
Speaker:it does,
Speaker:-has room for complementary support? -Conventional care is superb at structural
Speaker:problems, surgery, pressure control, injections.
Speaker:Where patients sometimes feel less supported is the chronic quality-of-life
Speaker:layer, like persistent dry eye or general ocular resilience through lifestyle.
Speaker:That's the space where nutrition, sleep, systemic inflammation, and possibly some
Speaker:-botanicals may help as a complement. -What would you most want to see happen in
Speaker:research over the next decade?
Speaker:Larger, well-designed, blinded human trials of specific standardized formulas
Speaker:for specific conditions, dry eye being the obvious candidate,
Speaker:and honest reporting of the null results too.
Speaker:The tradition doesn't need hype. It needs rigorous testing, and I'd rather know the
Speaker:truth than protect a nice story.
Speaker:For the educated patient listening,
Speaker:what's the takeaway when they weigh tradition against trials?
Speaker:Keep your ophthalmologist at the center.
Speaker:Treat traditional approaches as possible complements.
Speaker:Ask what evidence exists and how strong it is,
Speaker:and be skeptical of anyone promising cures.
Speaker:Curiosity and caution can live together, and that's exactly where I try to
Speaker:practice.
