Neuroprotective Strategies in Glaucoma: Current Evidence and Future Directions
Glaucoma is often described as a pressure disease, but at its core it's a disease of dying nerve cells. In this episode, Dr. Saikumar Gandapodi joins us to unpack what neuroprotection really means in glaucoma, why lowering eye pressure remains the foundation of care, and where the gaps in that approach open the door for adjunct strategies.

Speaker:Welcome back to the show. Today, we're talking about one of the most important
Speaker:and frustrating problems in eye care, glaucoma, and specifically the idea of
Speaker:neuroprotection.
Speaker:My guest is Dr. Saikumar Gandapodi, board-certified doctor of Oriental
Speaker:medicine, a practitioner of Ayurvedic medicine, and founder and director of the
Speaker:Netra Eye Institute. Dr. Gandapodi, thanks for being here.
Speaker:Thank you for having me. This is a topic I care about deeply because it sits right
Speaker:at the edge of what we can and can't yet do for patients.
Speaker:Let's start simple. Most people think of glaucoma as high eye pressure. Is that the
Speaker:right way to think about it?
Speaker:It's part of it, but not the whole story. Glaucoma is really a disease of the optic
Speaker:nerve, where the retinal ganglion cells slowly die off.
Speaker:High eye pressure is the biggest known risk factor and the only one we can
Speaker:reliably treat, but it's a risk factor, not the disease itself.
Speaker:That distinction matters, right? Because some people lose vision even with normal
Speaker:-pressure. -Exactly. We call that normal tension
Speaker:glaucoma, and it tells us something important. Lowering pressure helps, but
Speaker:for a subset of patients, the nerve keeps deteriorating anyway.
Speaker:That gap is what the whole field of neuroprotection is trying to address.
Speaker:So define neuroprotection for us. What does it actually mean here?
Speaker:Neuroprotection means protecting the retinal ganglion cells directly,
Speaker:independent of pressure.
Speaker:The goal is to keep those neurons alive and functioning longer, regardless of what
Speaker:the pressure is doing. It's a different target than the plumbing of the eye.
Speaker:Before we go deeper, tell me about your center and this approach you call Netra
Speaker:-Restoration Therapy. -At Netra Eye Institute, we practice
Speaker:integrative ophthalmology.
Speaker:Netra Restoration Therapy, or NRT, is our framework for supporting the eye through
Speaker:several biological pathways at once,
Speaker:things like ocular blood flow, mitochondrial function, oxidative stress,
Speaker:and inflammatory balance.
Speaker:I want to be very clear from the start, it's an adjunct. It complements standard
Speaker:glaucoma care. It doesn't replace it.
Speaker:I appreciate you saying that up front. Let's talk mechanism.
Speaker:Why do these ganglion cells die?
Speaker:What's happening biologically?
Speaker:There isn't one single cause, which is part of why it's hard.
Speaker:Elevated pressure creates mechanical stress at the optic nerve head,
Speaker:but we also see impaired blood flow, mitochondrial dysfunction, oxidative
Speaker:stress, and a decline in neurotrophic support.
Speaker:These processes feed on each other.
Speaker:Let's take those one at a time.
Speaker:Blood flow first.
Speaker:How solid is the evidence there?
Speaker:Reasonably solid as an association.
Speaker:Studies have consistently found reduced ocular blood flow and vascular
Speaker:dysregulation in many glaucoma patients, especially in normal tension cases. The
Speaker:harder question is causation, whether poor perfusion drives the damage or reflects
Speaker:it.
Speaker:It's probably both in a vicious cycle.
Speaker:There's this concept of vascular dysregulation,
Speaker:cold hands and feet, low blood pressure.
Speaker:Is that real in glaucoma?
Speaker:It's a recognized pattern, sometimes called Flammer syndrome,
Speaker:more common in normal tension glaucoma.
Speaker:These patients tend to have unstable blood flow and can be sensitive to dips in
Speaker:nighttime blood pressure.
Speaker:It's clinically observed and increasingly studied, though we shouldn't overstate how
Speaker:well we can treat it yet.
Speaker:What about mitochondria?
Speaker:-That's a word patients hear a lot now. -Retinal ganglion cells are metabolically
Speaker:hungry, especially at the optic nerve head, where the axons aren't yet
Speaker:insulated.
Speaker:That makes them very dependent on healthy mitochondria.
Speaker:Lab and animal work suggests mitochondrial dysfunction and the resulting oxidative
Speaker:stress contribute to their vulnerability.
Speaker:It's a compelling story mechanistically.
Speaker:That leads naturally to a supplement people ask about constantly,
Speaker:nicotinamide, vitamin B3.
Speaker:What does the evidence show?
Speaker:This is one of the more genuinely promising areas. In mouse models,
Speaker:high-dose nicotinamide protected retinal ganglion cells quite robustly by
Speaker:supporting their energy metabolism.
Speaker:In humans, small early trials have shown modest improvements in retinal function on
Speaker:electrophysiology testing,
Speaker:but these are short, small studies. We do not yet have long-term data showing it
Speaker:preserves vision over years.
Speaker:So promising, but not established.
Speaker:Right. I'd call it emerging and worth watching, not proven.
Speaker:And anyone considering high-dose nicotinamide should do it under medical
Speaker:supervision because dose matters, and it isn't risk-free.
Speaker:Let's talk about the drug that had the biggest neuroprotection expectations,
Speaker:memantine, and the whole excitotoxicity idea.
Speaker:That's an instructive story. The theory was that excess glutamate overexcites
Speaker:ganglion cells and kills them, and memantine blocks that pathway. It worked
Speaker:in the lab, but the large human clinical trial did not show a clear benefit over
Speaker:placebo for glaucoma.
Speaker:It’s a humbling reminder that mechanism in animals doesn’t guarantee results in
Speaker:people.
Speaker:That's a really honest example.
Speaker:Why do you think so many neuroprotection trials have struggled?
Speaker:A few reasons. Glaucoma progresses slowly, so trials need to be long and large to
Speaker:detect a difference.
Speaker:Our tools for measuring nerve health are improving but imperfect.
Speaker:And if the disease is truly multifactorial, hitting one target may not
Speaker:be enough.
Speaker:That last point is central to how I think about treatment.
Speaker:Say more about that, because it connects to your integrative philosophy.
Speaker:If several pathways drive cell death, blood flow, oxidative stress,
Speaker:mitochondrial strain, inflammation, then a single-target drug faces an uphill
Speaker:battle. That’s the rationale for multi-target approaches—not as magic, but
Speaker:as matching the complexity of the disease.
Speaker:The honest caveat is that multi-target is a hypothesis. It doesn’t automatically
Speaker:mean better outcomes.
Speaker:This is where herbal medicine comes into your world.
Speaker:How do you think about that scientifically, not as folk remedy?
Speaker:Through what’s called network pharmacology.
Speaker:A single herbal formula can contain dozens of compounds touching multiple pathways
Speaker:at once. Instead of one molecule, one target, you have many molecules nudging
Speaker:several systems.
Speaker:That framing makes traditional formulas scientifically interesting for a
Speaker:multifactorial disease like glaucoma.
Speaker:Is there any actual glaucoma evidence for specific botanicals?
Speaker:Some, though it’s early.
Speaker:Ginkgo biloba has small studies in normal-tension glaucoma suggesting
Speaker:possible effects on ocular blood flow and maybe visual field stability. But the
Speaker:trials are small and mixed.
Speaker:Saffron and certain other antioxidants have preliminary retinal data.
Speaker:I present these as candidates under study, not as established treatments.
Speaker:You practice Ayurveda as well.
Speaker:How do concepts like vata or rakta dhatu map onto this, if at all?
Speaker:I treat them as interpretive frameworks, not scientific equivalents.
Speaker:When Ayurveda describes a vata-type dysregulation of circulation and nerve
Speaker:function,
Speaker:I read that as a conceptual parallel to what we’d call vascular instability and
Speaker:neural vulnerability. It can guide how I assess a whole person,
Speaker:but I don’t pretend it’s the same thing as a measured biomarker.
Speaker:Same question for the gut-eye axis, which is trendy right now.
Speaker:Real or hype in glaucoma?
Speaker:Cautiously interesting.
Speaker:There’s early research linking the gut microbiome and systemic inflammation to
Speaker:neurodegeneration broadly,
Speaker:and a little glaucoma-specific work exploring inflammatory signaling at the
Speaker:optic nerve.
Speaker:But for glaucoma specifically, this is very much hypothesis-generating.
Speaker:I wouldn’t build a treatment plan around it as if it were settled.
Speaker:Let’s ground this.
Speaker:A patient is on their pressure-lowering drops, doing everything right.
Speaker:What does neurotrophin biology add to the picture?
Speaker:Neurotrophins like BDNF are survival signals for neurons.
Speaker:In glaucoma models, when pressure rises, the transport of these survival factors
Speaker:along the optic nerve gets disrupted,
Speaker:so the cells are essentially starved of support.
Speaker:Restoring that signaling protects cells in animals.
Speaker:Translating it to a practical human therapy has been much harder,
Speaker:and delivery to the eye is a real obstacle.
Speaker:So how does NRT actually fit alongside conventional care for a real patient?
Speaker:First, the patient stays on their prescribed pressure-lowering therapy and
Speaker:keeps seeing their ophthalmologist.
Speaker:That’s non-negotiable. Around that, NRT looks at modifiable contributors:
Speaker:blood flow, sleep and nocturnal blood pressure, exercise, nutrition, oxidative
Speaker:and inflammatory load, and where appropriate, evidence-informed supplements
Speaker:or botanicals.
Speaker:It’s supportive scaffolding, not a substitute.
Speaker:Are there lifestyle factors with genuinely decent evidence you emphasize?
Speaker:Aerobic exercise has reasonable data for supporting ocular perfusion and general
Speaker:vascular health.
Speaker:Avoiding steep drops in nighttime blood pressure matters,
Speaker:so I coordinate with physicians if someone’s on aggressive blood pressure
Speaker:medication at night.
Speaker:And not smoking, managing sleep apnea—these are unglamorous but real. They
Speaker:won’t cure glaucoma, but they address the terrain.
Speaker:What about the things patients try that you actively caution against?
Speaker:Stopping their drops because a supplement made them feel proactive—that’s the
Speaker:dangerous one.
Speaker:Also very high-dose supplements taken without oversight.
Speaker:And extreme practices—certain intense inverted yoga postures can raise eye
Speaker:pressure. Integrative care has to include knowing what can harm, not just what might
Speaker:help.
Speaker:Let's look forward.
Speaker:What emerging directions in neuroprotection genuinely excite you?
Speaker:A few.
Speaker:Nicotinamide and metabolic support are advancing into larger trials.
Speaker:Gene therapy and neuroregeneration research
Speaker:aiming to protect or even coax regrowth of ganglion cells is early but real.
Speaker:And better imaging and functional tests that let us detect change sooner, which
Speaker:would make it far easier to prove whether a neuroprotective strategy works.
Speaker:Is regenerating the optic nerve actually on the table,
Speaker:or is that science fiction?
Speaker:In the lab, researchers have gotten some retinal ganglion cell axons to regrow in
Speaker:animals, which was once thought impossible.
Speaker:That's genuinely exciting.
Speaker:But going from a mouse to restoring meaningful vision in a human is an
Speaker:enormous leap.
Speaker:I'd file it under real hope, distant timeline.
Speaker:If you could fix one blind spot in how glaucoma is managed today, what would it
Speaker:-be? -I'd want us to treat the patient's whole
Speaker:physiology, not just the pressure number.
Speaker:Conventional care does the pressure part very well, and it saves sight.
Speaker:What it's less set up to do is systematically address blood flow and
Speaker:metabolic resilience.
Speaker:That's the space where thoughtful, integrative care can add value,
Speaker:honestly and alongside standard treatment.
Speaker:For the educated patient listening,
Speaker:what's the single most important takeaway?
Speaker:Keep your pressure controlled and stay adherent. That's the proven foundation,
Speaker:and it protects vision.
Speaker:Then ask smart questions about the other factors: blood flow, lifestyle, metabolic
Speaker:health.
Speaker:Be curious about neuroprotection, but demand evidence and be skeptical of anyone
Speaker:promising a cure.
Speaker:The good options are adjuncts, not replacements.
Speaker:How should someone evaluate a clinic or a supplement claiming neuroprotection?
Speaker:Ask what the evidence is and whether it's lab, animal, or human.
Speaker:Ask if it's meant to complement or replace their drops. The honest answer is always
Speaker:complement. And be wary of guarantees.
Speaker:Real practitioners tell you what we don't know as readily as what we do.
